Substantiation
Choice Strategies administrates tax-favorable plans governed by IRS guidelines. In order to keep the plans in compliance, and maintain qualified (tax free) status, the IRS stipulates that the plan sponsor (employer) ensure that plans are properly substantiated. In other words, expenses incurred must be proven to be eligible under the plan. Plans that are not properly substantiated are subject to loss of qualified status as well as to fines and penalties.
Choice Strategies recognizes that our employer clients and employee members’ money and time is valuable. That is why we offer a unique voluntary Substantiation Service that helps limit the number of requests for documentation sent to employees for medical transactions on their Choice Strategies card. Through this voluntary service, employees grant Choice Strategies access to review their health insurance account online. Choice Strategies views the EOBs necessary to substantiate the medial services paid for with the card. A documentation request for these transactions is not generated if the information from the health plan matches the transaction and if the services are eligible under the plan.

Signing up for the Substantiation Service is quick and easy. Click here for a copy of the HIPAA Release form.
To complete a HIPAA Release form:
- Please read the top section describing your rights as a health care consumer, and check the box authorizing Choice Strategies to look up your Explanation of Benefits (EOB) on your behalf.
- Fill out your basic contact information in the top section of the HIPAA Release form.
- You will need the Health Plan Member ID Number which is located on your member ID card from your insurance company
- Provide your health insurance username and password (This is different from Choice Strategies website login information). If you have not created an online account please do so from your primary health insurance website. Some health insurance companies require separate logins for spouse and dependents over the age of 18. If this is the case, you will also need to include your dependents’ usernames and passwords as well.
- Once completed, please return to Choice Strategies via:
